Fairview Health Services Care Management Coordinator in Burnsville, Minnesota
Fairview Health Services is searching for enthusiastic individuals looking for a rewarding career in our busy Care Management department at our Burnsville, Ridges campus. This is an 8 hour Day position, 8:00am to 4:30 pm and includes every other weekend rotation.
Overview of the department – FRH hospital is a 150 bed hospital with a 20 bed observation unit.
Our units serves a high acuity, diverse population of patients, the units include orthopedics, oncology, ICU, medical tele, Mom/baby and pediatrics patients. The department is an energetic, fast-paced, complex environment offering advanced learning opportunities for the right candidate while partnering with an experienced multidisciplinary team.
The Hospital Care Management Coordinator provides support to the Hospital Care Coordination RNs and Social Workers to assist in managing patient discharge needs across the continuum of care. Job responsibilities include performing administrative tasks, such as assisting with TCU placement referrals,, updating referral information, scheduling follow-up appointments and verifying information related to the patients plan of care and referral needs.
Perform secure referral management/analysis for various sources .
Each morning updates open referrals. Identify potential delay of discharge referrals.
Monitors incoming TCU Referrals mailbox.
Utilizes PHI/HIPPA protocol for all communication.
Use EPIC, Right Fax, Cisco Desktop with demonstrated efficiency to expedite sending SNF referrals.
Maintain data for Care Transitions Metrics, submit data to Care Transitions Manager.
Act as customer service representative for SNF referrals and Care Transitions team.
Serve as liaison for patient placement in SNF, for community/clinic SNF referrals and report customer service issues related to referral activity to Care Transitions Manager.
Assists with guidance from Care Coordinator RN and SW to deliver important information to patients.
Provides and tracks delivering Medicare important Message letters to appropriate patients meeting criteria.
Assists with documenting key patient information into EPIC and Plex
Assists with scheduling post hospital follow-up appointments
Assemble discharge folders for patients
Work well on a multi-disciplinary team
Excellent time-management skills
- Bachelor’s Degree or Associates degree in a health related area
Self-directed, able to work independently with minimal supervision
Demonstrated competence in setting and managing multiple priorities.
Demonstrated ability to work with a variety of customers.
Broad knowledge of health care delivery systems gained through education or related work experience.
Experience with word processing, database and Excel
