Fairview Health Services Patient Care Management Coordinator in Burnsville, Minnesota
Assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s), performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act.
Coordinates care with members of the health care team and delegates responsibilities to others as appropriate.
Provide comprehensive care transition management of patients along the health care continuum, as appropriate from referral and case finding.
Assess patient needs, evaluate progress, facilitate decision-making and collaborate with the health care team during the patient episode of care, as well as responsibility for creating a plan of care that mitigates potential adverse consequences and promotes engagement towards success of their health care goals.
Identification of resources, facilitation of options for movement along the care continuum, and identification of transition care needs.
Identify and resolves barriers which may hinder effective transitions.
This is a day shift position and includes every other weekend rotation. Weekend shifts will be 12 hour shifts/weekday shifts will be 8 hour shifts to total 48 hours every two weeks (.6FTE), and this position is benefit eligible
MN RN license by start date
5 years clinical RN experience in a hospital setting
Critical thinking skills
Case management background
One year care coordinator/care management experience
Public Health Nurse license
Work effectively with the healthcare team across the care continuum
extensive assessment skills both pathophysiological and psychosocial.
