Fairview Health Services Household Coordinator in Edina, Minnesota

Looking to start in position to support and advocate for residents on the unit with the goal of facilitating a smooth admission into the facility, provide person-centered care to meet the psychosocial needs of the resident, and coordinate a successful discharge plan for our short-term residents.

This Household Coordinator position includes;

80 hours every two weeks

Day shift, no weekends

  • Positively support the culture of the household (unit) by establishing and maintaining effective communication systems with all members of the interdisciplinary team.

  • Create an exceptional experience that prioritizes safety and service to improve the experience, health, and wellbeing of everyone.

  • Facilitate a smooth admission into the facility by completing intake assessment, admission paperwork, facilitating PAS screening and Level II assessments as needed.

  • Manage resident caseload by assessing resident psychosocial needs and developing care plans for cognition, psychosocial, mood, behavior, and discharge plans.

  • Assist with Advance Care planning conversations.

  • Serve as the resident’s advocate and oversee residents’ quality of life.

  • Complete BIMS, PHQ9 interviews, the MDS, and related documentation.

  • Facilitate and lead care conferences and interdisciplinary team meetings.

  • Identify, investigate, and work to resolve complaints and grievances on the unit.

  • Facilitate and manage discharge planning and coordination.

  • Performs other duties as assigned and directed.

Recommed Qualifications:

Strong written and verbal communication skills

Strong interpersonal and intrapersonal skills

Commitment to be engaged and a positive addition to the facility culture.

Knowledge of PAS, facility processes, intake assessments, payers, and paperwork

Knowledge of assessment and care planning process

Knowledge of regulations

Knowledge of Social Work Ethical standards of practice

Knowledge of community resources and the financial considerations for eligibility

Ability to be a team player
