Fairview Health Services Physical Therapist - Home Care and Hospice in Hibbing, Minnesota

Fairview Range is looking for a Physical Therapist to join our Hibbing Home Care and Hospice team.

As a dedicated Physical Therapist, we orchestrate the flawless delivery of physical therapy services tailored to the unique needs of clients spanning all age groups. Our approach involves meticulously crafting a comprehensive care plan, encompassing essential education and support for both clients and caregivers, continual progress monitoring, hands-on care provision, and resource management. Additionally, we provide invaluable supervision to paraprofessional staff and maintain open collaboration with the Supervisor/Manager to address pertinent issues.

Schedule: This position will work up to 40 hours per two weeks. The shift length will be 8-hours on the day shifts, with no weekends.

As a Physical Therapist, you will be responsible for:

  • Planning: Ongoing development and implementation of the plan of care.

  • Organizing/Staffing/Directing: Coordination of services.

  • Control: Maintains complete, accurate and timely clinical records/documentation on all clients according to agency policy.

  • Professional Development

Required Qualifications:

  • Current MN Physical Therapist licensure.

  • One year of acute care, long term care or clinic experience.

Preferred Qualifications:

  • Doctorate of Physical Therapy

  • One year experience in home care.

  • BLS
