CD Adult Evaluation department
The department consists of 3 full time SUD evaluators, one full time gambling counselor, and a supervisor. The department is tasked with performing scheduled SUD assessments from the community and as needed on the inpatient medical and psychiatric units. We see patients 18 or older of all socioeconomic classes, of all gender orientations or cultural backgrounds. Other duties include doing updated SUD evaluations for patients admitting to Lodging Plus and reviewing SUD evaluations from outside referrals for Lodging Plus. One of the evaluators with an MSW also conducts Medicare assessments for our Deaf and Hard of Hearing patients, supervises other staff seeking their LPCC and will soon begin doing 1 to 1 mental health sessions for patients on the LP.
The Adult/Adolescent CD Evaluation Counselor is to gather and assess sufficient CD data from adults/adolescents across Behavioral Services to make a valid CD assessment and make appropriate TX recommendations. The counselor is to be knowledgeable about third party payers and how to obtain the appropriate authorization for TX. The counselor is to follow Fairview’s ethical standards of practice
Bachelor's Degree in Human Services or relatied Field
1 year experience in chemical dependency field
Basic Life Support Certification
Master's Degree in Human Services or related field
