Fairview Health Services RN Admission Clinician - Hospice in Minneapolis, Minnesota


Fairview Health Services is looking for an RN Admission Clinician based out of the Minneapolis office, but will travel throughout the metro area. This position is a 0.8FTE with a work shift of Monday, Tuesday, Thursday and Friday from 12:00PM to 8:30PM and includes a weekend rotation.

Responsibilities/Job Description

The Hospice RN Admission Clinician explains the Fairview Hospice Program and services to patients and their families/caregivers, develops an individualized plan of care, related to the patients' needs, in collaboration with the physician, patient and significant others. Determines appropriate utilization of resources based on clinical assessment and HHRG guidelines.




  • One year recent RN experience


  • Licensed as a RN and eligible to practice in Minnesota

  • Current CPR certification - BLS for Health Care Providers



  • Bachelor’s degree in Nursing


  • Two or more years RN experience in oncology, med/surg, critical care of high tech home care/hospice


  • Public Health Nurse (PHN) certification

Additional Requirements:

  • Reliable transportation

  • Valid Minnesota driver's license (or license that matches your current permanent residence)

  • Current and adequate automobile insurance policy

  • Computer skills: able to document effectively in EMR
