Fairview Health Services Social Worker - MSW in Minneapolis, Minnesota
This position is to provide social work coverage to patients and families who are requiring care at the University of Minnesota Medical Center and/or the University of Minnesota Masonic Children's Hospital. The position is to provide coverage on various units on weekends.
Provide emotional support, crisis intervention and counselingto pediatrics, adults, and older adults with social and emotional problems related to hospitalization, illness, disability or trauma.Identify and work to remove, potential barriers to success for the patient and/or family.
Psychosocial assessments, information and referral; advocacy, financial assistance/resources and outside referrals for ongoing supportive services and case management.
Initiate discharge planning and work with community partners (nursing homes, hospice, insurance/clinic care coordination, waivered programs) in the transitioning of patients to the next level of care in the community.
Minimum work requirement is two weekend shifts or two weekday shfits each month (8-4:30) and one weekend around a holiday each calendar year as well. Opportunity for additional weekday shifts and after-hours pager coverage is also available.
Master's degree in Social Work
Current LGSW required
Previous health care experience
Knowledge around chemical health, mental health, insurance programs, elder abuse, child neglect/abuse, and intimate partner violence
