Fairview Health Services Physical Therapist - Outpatient Pediatrics in Princeton, Minnesota

Overview

We are seeking a collaborative, outpatient pediatric physical therapist at M Health Fairview Rehab Services Northland Medical Center! The pediatric patient population consists of general practice pediatrics, including both orthopedic and neurological diagnoses. Our interdisciplinary pediatric team includes Physical Therapy, Occupational Therapy and Speech Language Pathology. We also have Orthotics and Prosthetics on-site for collaboration for pediatric needs. The pediatric space includes private treatment rooms, and a large open gym space with climbing wall, clubhouse, and ceiling attachment for a variety of swings. If pediatric caseload fluctuates, expectation is to support adult volumes for general practice outpatient clinic.

  • FTE .075, authorized for 60 hours per pay period.

  • Weekend rotation for acute care support for general medical and orthopedic patients on the medical/surgical floor (rotation shared across 15 physical therapists during the year).

M Health Fairview Rehabilitation offers a broad range of services that serve patients across ten acute-care hospitals, three post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities Job Description

The Physical Therapist partners with the patient and their care providers while performing evaluation and re-evaluation. Captures and applies pertinent data, performs examination, and determines treatment diagnosis

Develops an individualized plan of care through collaboration with the patient and family

Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals

Implements and modifies care plan based on reassessment and patient response

Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan

Partners with patients and families to provide skilled intervention

Implements and alters treatment to reflect change in patient status and response to treatment

Assesses patient and/or caregiver learning needs and provides appropriate education

Documentation/Billing

Uses acceptable medical terminology and follows professional practice standards.

Professional Expectations

Prioritizes tasks and patient care for effective time management.

Completes documentation in a timely manner per department standards

Stays in sync with department billing standards

Meets or exceeds departmental standards for productivity

Identifies opportunities for process /quality improvement

Qualifications

Required License/Certification/Registration

Physical Therapy registration/licensure in the state of Minnesota or temporary license in MN

Current Basic Life Support (BLS) certification

Preferred Education/Experience

Doctorate degree in Physical Therapy

1+ year of Physical Therapy practice
