Fairview Health Services Hospice Aide in Saint Paul, Minnesota
Fairview Health East - Home Care and Hospice Services is looking for a caring individual to join our team as a Hospice Aide to support our East metro region in the Minnesota. The hospice aides will provide assistance with personal cares the client is unable to perform independently, allowing the patient to be as independent as able. Along with physical support aides are there to provide emotional support for the patient and family during this time of change. This position requires the aide to travel from home to home (home being where patient lives and could be a skilled nursing facility, assisted living facility or their own home).
This position is 0.8 FTE or 64 hours in 2 weeks and benefit eligible. Monday -Friday, typical shift is 730-400, but will work occasional on weekends and will be in a holiday rotation. Mileage reimbursement per IRS guidelines.
The home base will be at our St Paul Office and will support cities in the East Metro Region of Twin Cities, MN
Performs delegated patient care activities in the home as outlined in the Hospice Plan of Care. Provides total patient care asspecified in Nursing Assistant/Hospice Aide job description for caseloads of patients in their homes.
Communicates appropriate information to members of Interdisciplinary Team. Reports patient/family information to appropriate IDGteam members. Documents patient/family care provided in the home. Recognizes and reports significant changes in patient condition.
Participates in educational requirements to meet Hospice Conditions of Participation. Attends a minimum of 12 hours of educationalactivities per calendar year; maintains accurate record of educational activities.
Promotes safe environment for patient/family in the home setting. Recognizes and responds appropriately to unsafe situations in thehome.
Responsible for competency training required but not limited to the State Operations Manual Conditions of Participation section 418.
Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.
Actively participates in creating and implementing improvements.
Performs other responsibilities as needed/assigned.
Must have basic ability to use computer and applications that are associated with performing basic work tasks,i.e. electronic medical health records
Interpersonal communication skills are required to maintain rapport with coworkers, families, patients while actively listing to develop a position connection
Experience: 6 months nursing assistant or hospice aide experience within the past 2 years.
Must be a Registered Nursing Assistant in MN (NAR)
Valid MN Driver's license, current auto mobile drivers licensure and reliable transportation
BLS for healthcare providers
Hospice home health aide experience
certificaton as HHA or nursing assistant registry
