Overview

In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. Located at Fairview Lakes Medical Center /Hospital in Wyoming, MN,

• Casual PT position serving adults primarily in an IP Rehab setting.

• Seeking Edema therapist to support current veteran Edema team (PT and PTA) with volume support and vacation coverage.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Responsibilities Job Description

The Lymphedema Therapist partners with the patient and family while performing Evaluation/Re-evaluation & Develops an individualized plan of care through coordination

Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals

Implements and modifies care plan based on reassessment and patient response

Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan

Partners care team to provide skilled intervention

Implements and alters treatment to reflect change in patient status and response to treatment

Assesses patient and/or caregiver learning needs and provides appropriate education

Documentation/Billing

Uses acceptable medical terminology and follows professional practice standards.

Completes documentation in a timely manner per department standards

Stays in sync with department billing standards

Professional Expectations

Prioritizes tasks and patient care for effective time management.

Understands and maintains licensure/certification requirements

Advises team members and/or supervises students as designated

Pursues professional development to support clinical standard processes

Quality Improvement

Crafting and implementing improvements, and participates in and supports entity/system initiatives

Qualifications

Required

• Physical Therapy (PT) licensure in the State of Minnesota

Preferred

Education

• Advanced Degree in PT

• Certified Complete Decongestive Therapist (examples: Guenter Klose (Lemer Lymphedema Services), Chikly, Casley-Smith, Vodder, Norton School of Lymphatic studies etc. and/or the Lymphedema Association of North America (LANA)

Experience

• 1+year Lymphedema Therapy experience
