Fairview Health Services RN Patient Care Management Coordinator in Wyoming, Minnesota
Fairview Health Services is searching for enthusiastic RNs looking for a rewarding career in the North Region Care Management departments at Northland Hospital in Princeton, and Lakes Hospital in Wyoming.
This is a fulltime, day position; 8 am to 4:30 pm
Every third weekend
Position is in the inpatient care transition/case management unit
This a regional position and must be willing to work at both Wyoming, MN and Princeton, MN
This position provides comprehensive care coordination of patients as assigned. The care coordinator assesses the patient’s plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient’s health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.
Responsibility to determine how to best accomplish functions within established procedures. Consults with manager/director on any unusual situations. Internal customers include all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External customers include physicians and their office staff, payers, community agencies, provider networks, and regulatory agencies.
BSN or BAN
MN RN License in good standing
5 years clinical experience
1-2 years working as a care coordinator/case manager
An understanding of hospital, community resources and resource/utilization management
Working knowledge of use of evidence based guidelines
Demonstrate ability to work effectively on an interdisciplinary team
Familiarity with computer systems and Microsoft applications
Ability to work in Wyoming, MN and Princeton MN as well as having flexible hours, including covering weekends
Demonstrate critical thinking skills, problem-solving abilities, effective communication skills and time management skills
Case Management Certification
Excellent customer service and communication with attention to detail
